TERMS AND CONDITIONS
The parent/guardian is responsible for full payment of all fees and charges, even if childcare is paid for in full or in part by an employer, a government agency (including HRA or ACD), or any other party.
I understand that my co-pay is due in advance, on the Monday of the week for

I understand that if my co-pay is not current, my child may not attend until me The account is paid in full.
I understand that my co-pay is due even if the child is absent from the program for Any reason.
I understand that my voucher covers my attendance for the days and times agreed on above.
I understand if I exceed the time allotted by my voucher that I must pay the required fees at the time of occurrence, or my child may not attend until my Account is current.
I understand that Mariela Eduardo at First Bloom Learning Center is required by law to notify the proper authorities if you are not current with your account and my voucher could be revoked for no compliance.
I understand that I must report any changes in status to ACS/HRA and the First Bloom Learning Center immediately.
I understand that I must immediately notify the school of any change in co-pay.
I understand that if my voucher ends, and I wish to remain in the school, I am Responsible for all tuition and fees.
Parents will be required to provide their own childcare if I (the provider) am unable to do so due to illness, closings, or scheduled vacations.
Parent vacations: Each family will receive 2 weeks’ vacation tuition free per year. If you plan on keeping your child out of care for longer than 2 weeks, see extended absence. Vacations weeks must be taken together not separated into days. Vacations only apply to families with deposits.
Provider vacations: First Bloom Group Family Day Care staff will take up to 2 weeks for vacation every year. Parents will receive at least 30 days' advance written notice of the exact dates.
Either the parent or provider has the right to terminate service for any reason, providing a 2-week written notice is given.
Collections : If a period of one week (seven days including weekends) after the time to which child last attended the contract may be terminated and the position filled.
The client will be responsible for any payments owed up to this point, any late fees that accrue and also for the time of the child’s required two-week notice even if the child has not be present in the daycare.
The client will also forfeit the security deposit. It will not be applied to the account balance.
You agree to reimburse us the fees of any collection agency, which may be based on a the percentage at a maximum of 35% of the debt, and all costs, and expenses, including reasonably attorneys’ fees, we incur in such collection efforts.
The collection agency that I will be using is IC System Premier Collection. The client’s account will also be reported to Provider Watch, a national child care credit reporting agency catered for child care providers to run checks on clients.

This contract will be revised on a needed basis
By signing this contract, the parents/guardians also agree to abide by the provider’s written policies. These written policies may be changed periodically at the discretion of the provider. The provider agrees to provide parents/guardians with two weeks' written notice of any changes.
ACD 1301 Voucher
If you have this form, make sure the form is not expired. Sign Part. 1 and email the form to us at firstbloomdaycare@gmail.com. Once we complete our Parts, I will email the form to priority.voucher@acs.nyc.gov and CC you in the email. We should receive a response within 48hrs.
